Statement of Privacy Policy and Data Retention in Compliance with General Data Protection Regulation (GDPR, AVG in Dutch)
As the sole practitioner of the private practice operating under the name of BraveKey Therapy, I am committed to following the Nederlands Institute van Psycholoogen (NIP) ethical guidelines for confidentiality and also to protecting electronic data collected via our contact in the practice. The website of the practice is www.bravekeytherapy.nl.
Personal data is generally limited to contact information provided by clients as part of the intake such as name, address, or phone number. This information is used for direct contact and billing purposes only and is not shared with third parties. Contact with a third party, such as an insurance company would be initiated at the request of the client and any communication would be previewed by the client themselves. Any information stored on the practice computer or cell phone is encrypted and password protected. Written notes or communication are stored in a locked cabinet.
Retention of any data collected as part of the operation of the practice follows general guidelines of NIP. Medical data is retained for 15 years after termination, financial information is retained for 7 years for tax purposes.
The website www.bravekeytherapy.nl is SSL certified and anonymous visitor information is collected by the website to track number of visitors, duration of visit and pages viewed.
You have the right to request that your information be deleted at any time. In the event of a digital data breach, you will be informed in a timely manner.
In principle, your information will not be transferred to any country outside the European Union. Should this be necessary at your request, all efforts will be made to insure that the country in question offers an adequate level of protection in compliance with the AVG regulation.
Changes may be made to this privacy policy at any time and a current version will be published on the website.
